In order to confirm that your product is part of the 2023 voluntary recall and to enable us to process your claim as quickly as possible we require the following information:

The recalled bottle(s) with the UPC code (Barcode) visible. (If submitting multiple bottles please take an extra picture all the bottles in total)

Manufacturing (lot) code for each of the recalled bottles. (Affected codes are those whose first eight numbers begins with 2345US78 through 2365US78 or 3001US78 through 3023US78)

Once we confirm the product from your photos we will issue you a reimbursement (4-6 weeks). Please dispose of the product in its container with household trash. Do not empty the product prior to disposal.
